MEDICATION DURING SCHOOL HOURS

Any student who needs to take medication prescribed by his/her physician during the school day must have on file with the school the form entitled Medication During School Hours from his/her physician.  This form must be completed before any student will be allowed to bring or to take any medication at school.  The form is available from the school office.  The medication must be sent in the container with the prescription number and the doctor’s name.  All medications must be stored in the school office and administered by school personnel.  Non-prescription medication (cough drops, etc) can be administered by the staff if the “Medication During School” form has been signed by a doctor and is on file in the office.
